The main issues in these three vessels are the scaffold erecting could not meet the requirement and the housekeeping condition is not good, stil

It was assumed that the big hole maybe opened for the lifting job performed in engine room,
4、after the lifting work, the scaffolding platform shall be fixed completed immediately. At the same time, the worker was allowed to stand on this unqualified scaffold to carry out work (Figure 1). The same location, scaffold erected upper that scaffold needs to be improved for a big step. This scaffo
5、ld seems to be used for the pipes checking and also needs somebody standing on it, there was not guardrails installed for this scaffold and also the scaffolding platform was not covered carefully (Figure 2). 机舱上面脚手架缺少大的脚手架平台，脚手架平台只是覆盖了一半。机舱内打开的大孔看起来是为了起重作业，起重作业后，脚手架平台应该立即铺满跳板。同时，员工被允许站在这种不合格的脚手架上作业（

For the further investigation, the main access had been destroyed, through the grinding area was not allowed to get in without permission, it

11、 that the entry for getting in cargo room was lacking of safeguard protection, this week, the simple triangle safeguard was installed there. This kind of safeguard could not be accepted for the big holes protection, it is not safe enough to prevent people dropping down (Figure 7). 在主甲板，前期的问题是通往货舱的入口

The broken movable scaffold shall be removed and lots of cables were piled on the floor, these cables need to be hung up (Figure 8). 主配室附近5S情况应该保持更清洁。已经损坏的移动式脚手
13、架应该移除，大量电缆线堆放在地面上，电缆需要架空（图8） 。One piece of plank scaffold put on the machine or flange as the working platform in engine room, this kind of scaffold was not accepted to be used on jobsite, they must be removed immediately, this issue was found last patrol about 20# vessel, it still in there without
